MATCH REPORT PROCEDURES
At the start of the game collect a match report from each coach.
Check the form to make sure the identifying information (game date,
number, team information) is complete, that all players are listed
by name and number, and that there are notes for any player playing
up, playing across, or serving a suspension. The coaches do not need
to sign the reports at this time. Absolutely, under no condition, do
not start the game until you have both reports!! Delay the start of
the game if required. Note that the match reports should indicate
team colour, and whether they are home/away.
At the end of the game, stand near the centre of the field, off to
the side during the team handshake, and while over-seeing the
handshake complete the Match Report:
· Note the score on each Match Report (make sure each
report has the same score!)
· Make a brief note on any situations that should be
brought to the attention of the Head Referee, the Club, or the League.
Briefly note the name and number of any individual receiving a yellow
or red card, as well as any Special Incident report. Make the same
notes on each Match Report.
· Sign each Match Report.
· After the handshake each coach will verify the score,
read your notes, and sign each report.
· Hand the completed/signed Match Report back to all
coaches. It is the coaches' responsibility to hand in the Match Reports.
If you issued a Yellow Card, a Red Card, or have a situation worthy of a
Special Incident report, you must hand in an additional report within 48
hours after the completion of the game. Please check game sheet for fax
